FOR PATIENTS COVERED BY INSURANCE
For your convenience, Pro Vision Anesthesia Services will bill most insurance plans for the cost of anesthesia for your procedure.
Pro Vision Anesthesia requires a deposit of $500.00 when insurance is billed. The deposit is due prior to the start of the anesthetic. After payment is made by your insurance company, Pro Vision Anesthesia will refund to you any amount received in excess of the total billed.
Some offices "Bundle" the cost for a particular procedure, including the cost for anesthesia. In this situation, Pro Vision Anesthesia does not bill insurance, and is paid by the surgeon (i.e. Plastic Surgery).
Please refer to our Billing Agreement for more specific information.

Patients offered anesthesia services, but not opting to pay the cash pay rate must select Option 3 on the ABN acknowledging they do not want the anesthesia services offered by Pro Vision Anesthesia. With Option 3 you cannot appeal to see if Medicare will pay.
FOR CASH PAY PATIENTS

Payment in full is due prior to the start of the anesthetic.
During your pre-anesthesia evaluation you will be provided the total cost based on procedure information provided by the surgeon (procedure and length of case). If your procedure takes longer than the surgeon's estimate, there may be additional anesthesia costs at the completion of the procedure.
